| The
Clinical Palliative Care MDT is a fully integrated team with representatives
from hospital, primary care, social services and the hospice. It meets
weekly at the Hospice. Membership includes Macmillan nurses, specialist
cancer nurses, the Macmillan physiotherapist and palliative care hospital
physiotherapist, the hospital lymphoedema nurse, hospital and community
social workers, representatives with a special interest in palliative
care from occupational therapy, dietetics, podiatry, pharmacy and
speech therapy. Also represented are the transfer of care service
and district nursing services. From the Hospice the Medical Director,
Matron, day care unit Sister and Chaplain are members.
The
MDT provides input into Hospice admissions, discusses individual
patient’s care programmes and reviews problem cases and critical
incidents.
